My Safety Plan
This worksheet helps students build a personalized safety plan by identifying their boundaries, warning signs, support network, and safe places. Students fill in sections describing what makes them uncomfortable, physical or emotional cues that something is wrong, and trusted adults they can contact. The activity also asks learners to write safety steps they can take during pressured or unsafe situations, along with clear boundary-setting statements they can use. By completing this plan, students gain practical tools for recognizing danger, seeking help, and asserting themselves. The final section allows students to list important emergency contacts, reinforcing the importance of knowing who to reach out to in urgent situations. This worksheet strengthens self-advocacy, awareness, and preparedness.
